1. Participation Acknowledgement
I give permission for my child listed above to participate in Art Kids Summer Art Camp programs and activities, including but not limited to painting, drawing, sculpture, ceramics, mixed media, outdoor art activities, and other creative projects.
I understand that participation in art-related activities involves the use of materials, tools, and equipment that may pose a risk of minor injury (including but not limited to cuts, allergic reactions, or accidental spills), and I voluntarily assume all such risks.
2. Release of Liability
In consideration of my child being permitted to participate in Art Kids Summer Art Camp, I hereby release, waive, discharge, and hold harmless Art Kids Summer Art Camp, its owner, instructors, staff, volunteers, and any affiliated parties from any and all claims, demands, causes of action, or liability arising out of or related to my child’s participation, including any injury, loss, or damage, except in cases of gross negligence or intentional misconduct.
3. Medical Consent
In the event of an emergency and I cannot be reached, I authorize Art Kids Summer Art Camp staff to obtain any necessary medical treatment for my child, including first aid, emergency medical services, or hospital care.
I understand that I am financially responsible for any medical treatment my child may receive.
By signing this document, I acknowledge the inherent risks and voluntarily assume full responsibility for any injury, damage, or loss that may result from my participation. I hereby waive and release the business, its owners, and its staff from any and all liability, past, present, and future, relating to the services provided.
